For new patients please contact your previous eye doctor and have your records faxed over to us prior to your visit. Our fax number is 603-926-9546

• If you are a contact lens wearer, please wear your contacts to your visit.

• Bring any pairs of glasses and sunglasses you currently use.

• Bring your insurance cards to the appointment. Call your insurance company to make sure we are in your insurance plans network and know what your benefits are.
